I.       Personal Data

A.    Name, address, phone, high school data, community college data (if applicable)

B.    Employment History (if any) and State any special skills and usage

II.    Financial Need

A.    Enclose statement for Financial Aid Dept. of SIU

B.    State any other sources of financial aid

III. Academic History

A.    List all colleges and universities attended and give current over-all GPA

B.    Have an official transcript mailed from each school attended

IV.  Past and Current association membership

A.    List names of volunteer, school, religious, social, athletic, civic activities or organizations.

B.    Include the dates of participation and any office held within the organization

V.    Miscellaneous

A.    Include a short (not to exceed 1 page) statement as to the reasons you should be awarded this scholarship by Nubian Connection

B.    Show proof of admission into SIU.
